Skilled Nursing
Our skilled nurses are integral to the home health team. In most cases, the nurse will be the first to make the visit meeting with the patient and caregiver. On that initial assessment, the nurse will assess, evaluate, and determine the needs for home health. Once the start of care begins the home health nurse will continue to follow up with visits by assessing, teaching, implementing the plan of care, and providing skilled care.
Some of the skilled nursing services include:
- Teaching of the disease process and medication use
- Diabetes teaching & management, use of a glucose meter and insulin
- Heart disease/ heart failure management, we dedicate a Home Heart Team for this
- Pain control and management
- Care of a loved one that is bedbound
- Fall risk assessment, safety, and prevention
- Dietary and nutritional needs
- Ostomy care & urinary catheter care
- Post-surgical care (Orthopedic, post-knee or hip replacements)
- PT/INR monitoring for coumadin therapy
- Obtaining lab draws and specimens for testing ordered by your physician
- Encouragement and counseling
- Supervisory visits for the home health aide
The skilled nurse coordinates with all members of the home health team and will evaluate needs at discharge from home health also making the last home visit.

- Collaborating with Wound Clinics
- Diabetic Wounds
- Stasis Wounds
- Venus Ulcers
- Wound care and/or wound-vac (negative pressure wound therapy)
