For Providers

Dear Providers:

Today’s Health Care is ever changing. Due to new federal health care laws and regulations set forth by the Patient Protection and Affordable Care Act or also known as “Obamacare,” we providers are challenged and set with a to-do list to accomplish that is planned out for years to come. Physician or In-Patient Facility, we are all aware of this and have already implemented some of these changes already. There is hospital value-based purchasing, use of evidence based practices to improve patient care quality, and thus patient care outcomes and measures improved or maintained; or face penalties of decreased reimbursement that may compound yearly if improvements are not shown. Hospital readmission rates will be an upcoming topic, and to help improve the patient care continuum from an in-patient discharge transition back home, Assured Home Healthcare is aware of your important concerns. Whether a patient discharged and diagnosed with pneumonia, myocardial infarction, or congested heart failure; we at Assured Home Healthcare realize that there are still interventions in the home setting that prevent unnecessary hospital readmissions. If your are a physician or inpatient facility and looking for a partnership in a community based transitions program, Assured Home Healthcare is willing to help and take on that responsibility.

Sincerely,

Andrew Carlay RN, BSN

Clinical Coordinator at Assured Home Healthcare
